In-memory SkillStore backed by a thread-safe map.

Suitable for tests and embedded use cases where skills are wired programmatically rather than loaded from disk. Lookups are case-insensitive on skill name to match the Claude Code convention (the directory under <root>/ acts as the name and is conventionally lowercase).
